University of Nevada, Las Vegas International Student Insurance Waiver

Insurance Requirement

All University of Nevada, Las Vegas (UNLV) International students with F-1 visa status on a UNLV I-20 are required to have adequate medical insurance coverage. All International students are automatically enrolled in the UNLV-sponsored Student Health Insurance Plan unless they are eligible and choose to submit an online insurance waiver of comparable coverage. Waivers must be submitted each term. International student accounts will be charged the Student Health Insurance Fee for the Fall and Spring/Summer term.

Student Health Insurance is an affordable health insurance plan tailored to the health care needs of University of Nevada, Las Vegas students. It features year-round, world-wide coverage using the Cigna network. This means students can receive convenient, high quality care when they are sick or injured without having to be concerned about cost or availability of the care they need. The Student Health Insurance plan provides optimal coverage for services on campus and in the University of Nevada, Las Vegas community in addition to peace of mind for both parents and students.

Considering waiving out of Student Health Insurance?

If you already have a health insurance plan in place before the beginning of the term and you plan on waiving out of the UNLV Student Health Insurance Plan, you should first make sure the insurance plan you carry provides coverage within a 25 mile radius of the University of Nevada, Las Vegas area as some insurance plans (such as Nevada Health Plan) may restrict you to a provider network outside the UNLV area where you will be spending most of your time while attending school.

If you have verified that network coverage is available within the University of Nevada, Las Vegas area, please be prepared to answer the questions listed below. If you do not have the information to answer the questions below, you should contact your insurance company to collect this information prior to proceeding.

Be prepared to potentially respond to the following questions:

  1. Name of your current insurance company
  2. Phone # of your current insurance company
  3. Current group and medical ID# of your insurance plan
  4. Does your insurance plan have offices and operate in the United States?
  5. Does your current plan provide Primary Care services within 25 miles of the main UNLV campus?
  6. Does your current plan provide Emergency Care within 25 miles of the main UNLV campus?
  7. How much does your current plan pay for Inpatient Hospitalization?
  8. Does your current plan have Inpatient and Outpatient Mental Health benefits?
  9. What is your current plan’s individual annual deductible?
  10. Does your current plan provide Prescription Medication coverage?
  11. Does your plan have coverage for Medical Evacuation and Repatriation?
  12. Does your plan have an annual benefit maximum?
  13. Does your plan require you to pay in full for covered services and then file a claim to be reimbursed?
  14. You will need to upload an image of your current medical insurance card or benefit summary to complete the waiver .

If you are sure you want to waive, please have your health insurance policy information available and proceed to the online waiver form. NOTE: After logging in with your Student ID, the system will log you out of this waiver form if the computer is at rest for fifteen minutes or longer and you will have to begin again.

REMEMBER! Your waiver is valid for ONLY one semester. It is your responsibility to apply for health insurance waiver at the beginning of each semester (Fall & Spring/Summer). If approved, it takes 5 business days for the health insurance fee to be removed from your student account. The Health Insurance fee MAY NOT be removed if a waiver request is received or processed after the deadline to pay your tuition and fees without a penalty. You may also incur the university’s standard late fees. The waiver is effective for the current semester only. If you are no longer in F-1 visa status at UNLV, you will not be automatically enrolled in the Student Health Insurance in future semesters.
